Sudden acceleration on Jeep Cherokee:Specifically 1990 model

I am a police officer/accident reconstructionsist. I have recently experienced a tragedy in my family resulting in the death of my aunt. The incident involved a sudden acceleration of a 1990 Jeep Cherokee. I have come across information that there is a potential problem with the position of the accelerator and brake pedals in these vehicles. It seems that there is a high number of incidents involving Jeep Cherokees - 1996 and prior models. New models are equipped with a shift interlock device which prevents shifting unless brake pedal is depressed.
I am interested if anyone has experienced any similar incidents. Has anyone investigated crashes where the driver insists that he/she had his/her foot on the brake but the vehicle would not stop?
